Revival RN accepts cash and all major credit cards including MasterCard, Visa, American Express and Discover. We do not accept, personal checks, care credit or any type of health insurance. All payment is expected at time of service.

If you are more than 10 minutes late for your appointment we may need to reschedule you to be courteous to other clients. If this occurs, you will be charged a $50 reschedule fee.

Our service provider’s time is very valuable. If you cannot make your appointment for any reason, we understand. Simply notify us 24 hours in advance and we will be happy to get you rescheduled. A cancellation fee of $50 will be charged to your card for any appointments canceled within 24 hours. A fee of $150, or the cost of the scheduled service, (whichever is less) will be charged for a no show.

Your credit card will be placed on file at time of scheduling and will only be charged if this policy is broken.

Botox

BOTOX® Cosmetic (onabotulinumtoxinA) targets one of the underlying causes of frown lines and crow’s feet—the repeated muscle contractions from frowning and squinting over the years. BOTOX® Cosmetic temporarily reduces this muscle activity so you’ll begin to notice a visible smoothing of these lines. For temporary improvement of moderate to severe crow’s feet, your doctor will inject BOTOX® Cosmetic into the muscle surrounding the sides of your eyes—called the orbicularis oculi muscles. For temporary improvement of moderate to severe frown lines, your doctor will inject BOTOX® Cosmetic into 2 muscles—the corrugator and procerus muscles.

The treatment takes approximately 10 minutes and requires minimal downtime or recovery—it’s often called a “lunchtime procedure.”

You may begin to notice results within 24 to 48 hours for moderate to severe frown lines, with results lasting up to 4 months.
After your treatment, you can resume your day. There is minimal downtime and your specialist may give you specific aftercare instructions.
BOTOX® Cosmetic is a technique-sensitive treatment. You should not lose the ability to show expression when you are treated by someone who is licensed, trained, and a medical expert in facial anatomy. It is important to talk to your specialist about the results you want from treatment.
Yes. BOTOX® Cosmetic has also been clinically evaluated in male patients. In clinical studies for moderate to severe frown lines, 17.5% of patients treated with BOTOX® Cosmetic were men. In the clinical studies for moderate to severe crow’s feet, male patients comprised approximately 10% of patients treated with BOTOX® Cosmetic. According to the American Society for Aesthetic Plastic Surgery, 10% of all cosmetic procedures performed in the United States in 2014 were on men—that is more than 1 million cosmetic procedures. The number of cosmetic procedures for men increased by 273% from 1997 to 2014.
Some patients report that being injected with BOTOX ® Cosmetic (onabotulinumtoxinA) feels like a pinch. Your provider may use ice to numb the treatment area. If you are concerned about discomfort, your provider may apply a topical numbing cream before administering your treatment.

Juvederm

JUVÉDERM® is the #1 selling collection of hyaluronic acid fillers in the US. Each product in the JUVÉDERM® collection of fillers adds volume to a different area of the face – without surgery. All JUVÉDERM® fillers are a smooth, crystal-clear gel that are administered by injection in your aesthetic healthcare provider’s office.

The JUVÉDERM® collection of fillers is made fom hyaluronic acid (HA). HA is a natural substance in the skin that delivers nutrients, helps the skin retain moisture and softness, and adds volume. Each product in the JUVÉDERM® collection of fillers is made from a modified form of HA.

Your aesthetic healthcare provider will take your medical history and provide a facial analysis. Be prepared to discuss your aesthetic areas of concern. This is not a surgical treatment. Actual treatment with JUVÉDERM® injectable gels can take 15 minutes to 60 minutes, depending on which area of your face is being treated.

Different formulations are approved to either lift cheeks, smooth parentheses lines, smooth vertical lip lines or plump the lips. The results are subtle and long lasting.
JUVÉDERM VOLLURE™ XC softens moderate to severe parentheses lines and wrinkles by adding subtle volume around the nose and mouth in adults over the age of 21.

JUVÉDERM VOLUMA® XC adds volume to the cheek area to correct age-related volume loss in adults over 21.

JUVÉDERM VOLBELLA® XC adds subtle volume to lips and softens the appearance of vertical lip lines.

JUVÉDERM® XC smooths moderate to severe parentheses lines and wrinkles around your nose and mouth.

JUVÉDERM® Ultra XC adds fullness and plumps lips as well as soothes and corrects moderate to severe parentheses lines in adults.

Kybella

KYBELLA® is a prescription medicine and is the only FDA-approved injectable treatment used in adults to improve the appearance and profile of moderate to severe fat below the chin (submental fat), also called “double chin.”

It is not known if KYBELLA® is safe and effective for use outside of the submental area or in children less than 18 years of age.

What is submental fullness?
Submental fullness is a gathering of excess fat beneath the chin, commonly known as “double chin.” It can be something you’ve had your whole life due to genetics, or start to appear as you get older or if you gain weight.

The active ingredient in KYBELLA® is synthetic deoxycholic acid. Deoxycholic acid is a naturally occurring molecule in the body that aids in the breakdown and absorption of dietary fat. When injected into the fat beneath the chin, KYBELLA® destroys fat cells, and once destroyed, these cells can no longer store or accumulate fat. This results in a noticeable reduction in fullness under the chin, revealing an improved chin profile.
When injected into fat under the chin, KYBELLA® causes the destruction of fat cells. Once destroyed, those cells cannot store or accumulate fat. After reaching your desired aesthetic, further treatment is not expected.

B12 Injections

Vitamin B12 is a water soluble vitamin that plays an important role in many tasks such as forming red blood cells, maintaining the central nervous system, and other bodily functions. It is one of the most important vitamins because it performs essential functions like assisting in keeping a healthy blood flow and controlling key enzymes in the body. It is generally found in many similar foods and food products.
B12 can help with things like improving your metabolism, helping your memory, and boosting your focus and concentration. Vitamin B12 injections may help you detoxify your body, increase red blood cell production and maintain a healthy liver. It can also be used to aid with diabetes, male infertility, and osteoporosis, as well as, help prevent vitamin B12 deficiency.
Vitamin B12 deficiency occurs when there are low levels of Vitamin B12 in your blood. Some of the symptoms may include: depression, irritability, anemia, reduced heart function, and decreased fertility.
Cyanocobalamin is a man-made form of vitamin B12 used to treat low levels (deficiency) of this vitamin. Vitamin B12 helps your body use fat and carbohydrates for energy and make new protein. It is also important for normal blood, cells, and nerves.
The stomach acids that aid in the natural breakdown of food also breakdown vitamin B12. The body will only absorb a small amount of the nutrient from a pill. In addition, as you get older your body’s ability to absorb nutrients through digestion continually decreases. In fact, many adults are unable to absorb Vitamin B12 into their bodies at all! Injections provide a direct method of supplying your body with the necessary Vitamin B12.

IV Vitamin Therapy

If you’re like most people, you probably don’t consume your daily dose of vitamins and minerals, let alone experience their valuable benefits. Nutrient IV Therapy is a safe and effective way to receive natural vitamins, minerals, and amino acids direct to the bloodstream.
